The undersigned knowingly and fully assumes all risks, known and unknown, associated with participation in activities at JERSEY JUNGLE LTD indoor playground facility, and waives all claims for damage to personal property or injury to person arising from such participation.

I, and the children under my care, understand and agree to the safety rules and play guidelines.

RELEASE, DISCHARGE & AGREEMENT NOT TO SUE – I, for myself and any minor Participant child and our respective heirs, personal representatives and assigns, do hereby release, discharge and agree not to sue JERSEY JUNGLE LTD or its managers, members, employees and/or other agents, for any sickness, injury to or death of any Participant arising, directly or indirectly, from participation in the activities.

This release, discharge and covenant not to sue shall relate to any and all claims or legal rights now existing or arising in the future.

Indemnification And Hold Harmless – I agree to indemnify JERSEY JUNGLE LTD and hold JERSEY JUNGLE LTD harmless from, without limitation, any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees and costs, incurred due to claims brought by any party as a result of or arising out of my or my minor Participant child’s involvement in the Activities and agree to reimburse JERSEY JUNGLE LTD for any such costs, expenses and fees as they are incurred.

Security Cameras. I understand that security cameras installed around and within JERSEY JUNGLE LTD facility are for the safety of all participants and may be referenced should any incidents or injury occur.

Photo Release. I grant JERSEY JUNGLE LTD permission to use any photographs, video, or sound bites obtained of me or any minor Participant child at JERSEY JUNGLE LTD’s facility for any legal use including but not limited to: publicity, advertisement and web content.

I understand that no royalty or other compensation shall be payable to me or the minor Participant by reason of such use.

Parent or Legal Guardian Certification and Consent – I hereby certify that I am the parent or legal guardian of the minor Participant children whose name(s) appear below, and I have authority to waive rights on behalf of the minor Participant(s).

I have read and I understand all of the provisions of this document and the risks of the Activities.

This Participation Agreement, Liability Waiver, and Hold Harmless Agreement has no expiration date.

JERSEY JUNGLE INDOOR PLAYGROUND

PARTICIPATION AGREEMENT, LIABILITY WAIVER, AND HOLD HARMLESS AGREEMENT
The undersigned knowingly and fully assumes all risks, known and unknown, associated with participation in activities at Jersey Jungle Indoor Playground facility, and waives all claims for damage to personal property or injury to person arising from such participation. I, and the children under my care, understand and agree to the safety rules and play guidelines of the facility.

RELEASE, DISCHARGE AND AGREEMENT NOT TO SUE – I, for myself and on behalf of any minor participant child and our respective heirs, personal representatives and assigns, do hereby release, discharge and agree not to sue Jersey Jungle Indoor Playground, its owners, managers, members, employees and agents, for any sickness, injury, or death of any participant arising directly or indirectly from participation in the activities, including claims based on ordinary negligence to the extent permitted by state law. This release applies to any and all claims now existing or arising in the future during the term of this agreement.

INDEMNIFICATION AND HOLD HARMLESS – I agree to indemnify Jersey Jungle Indoor Playground and hold it harmless from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees and costs, incurred due to claims brought by any party as a result of or arising out of my or my minor participant child’s involvement in the activities.

CONTINUOUS USE – In consideration of not having to sign a new copy of this Agreement before each visit to Jersey Jungle Indoor Playground, I understand and agree that my execution of this Agreement authorizes Jersey Jungle Indoor Playground to apply this Agreement to all present and future visits, programs, birthday parties, and events attended by me and the minor participant during the twelve-month term of this Agreement, unless I revoke this Agreement in writing.

ACKNOWLEDGMENT – By executing this Agreement, I acknowledge that if I or the minor participant are injured, or if our property is damaged, in connection with our presence at or use of Jersey Jungle Indoor Playground, the participant and I may be found by a court of law to have waived our right to maintain a lawsuit against Jersey Jungle Indoor Playground and the released parties on the basis of any claim that is released in this Agreement.

SECURITY CAMERAS – I understand that security cameras installed around and within Jersey Jungle Indoor Playground facility are for the safety of all participants and may be reviewed should any incident or injury occur.

PHOTO RELEASE – I grant Jersey Jungle Indoor Playground permission to use any photographs, video, or audio recordings obtained of me or any minor participant child at the facility for lawful purposes including publicity and advertising.

AUTHORIZED SIGNER REQUIREMENT – I affirm that I am the parent or legal guardian of the minor participant and have authority to sign on the child’s behalf.

ANNUAL TERM OF VALIDITY – This Agreement is valid for twelve (12) months from the date of signature and applies to all visits during that period. After twelve (12) months, a new waiver must be completed.

I hereby confirm that I will supervise my child during their visit to Jersey Jungle and ensure their good behavior. I understand that I am responsible for monitoring my child's interactions with other children and ensuring they follow the rules. I also acknowledge that Jersey Jungle reserves the right to refuse service if they deem it necessary for the safety and well-being of other children. I have read and understood these terms and agree to abide by them.

I grant permission for our pictures to be taken. We understand that these images may be used for promotional or informational purposes, and we consent to their use without any further compensation or notification.
